Belial Overview
Belial can be found in Palace of the Deceiver, which is available on all Torment difficulties. The entrance is located in Amber Sands subzone in northern Kehjistan, just west of Tarsarak Waypoint.
If you have been planning an efficient boss route this season, you already know how important this location is. Belial sits at the very top of Lair Boss ladder. Most players do not go straight to him. Instead, they work their way up through the lower-tier bosses, collect the materials they need, and only challenge Belial once they are ready. Going in underprepared is usually a quick way to get deleted.
What makes Belial especially interesting is that you can fight him directly, but opening Belial's Hoard requires 2 Betrayer's Husks.
These do not come from regular enemies or standard farming zones. They drop from Mini Belial, a special encounter that has a low chance to appear after defeating Initiate or Greater Lair Bosses.
That means farming Belial efficiently is not just about beating him. It is about understanding the entire boss ladder below him and repeatedly running those encounters until Mini Belial shows up and drops the keys you need.

To farm Belial properly, you first need to understand the progression system that feeds into his fight.
Initiate Lair Bosses
- Varshan: 12 Malignant Hearts
- Grigoire: 12 Living Steel
- Beast in the Ice: 12 Distilled Fear
- Lord Zir: 12 Exquisite Blood
- Urivar: 12 Judicator's Masks
All Initiate Lair Bosses have a chance to drop Stygian Stones. These are especially valuable because they can be exchanged for the materials needed to unlock Greater Lair Bosses, including Shards of Agony, Pincushioned Dolls, and Abhorrent Hearts.
The one exception is Pounds of Flesh, a newer material that can only be earned by using a Bloodied Sigil to convert a standard encounter into a Bloodied version and then successfully clearing it.

Belial Strategy
Belial is dangerous for more than just his raw damage. The fight puts constant pressure on the player and heavily punishes hesitation or bad positioning. It is one of those encounters where one mistake can instantly snowball into a wipe.
Starting in Season 10, Belial cannot enter an invulnerable phase until 10 seconds have passed. For high-damage builds, this creates a major opening at the start of the fight. If your burst is strong enough, you can push him through key health thresholds before the fight becomes much more punishing.
- Belial gains shields when his health drops to two-thirds and one-third. Each shield lasts 5 seconds.
- The first shield, triggered at two-thirds health, absorbs damage equal to one-third of Belial's maximum life.
- The second shield, triggered at one-third health, absorbs damage equal to two-thirds of Belial's maximum life.
If you break these shields within the 5-second window, you can skip Belial's invulnerability phase entirely. That is the main damage check of the encounter. Builds with high burst, well-timed cooldowns, and strong group coordination can dramatically reduce downtime and make the fight much smoother.
On top of the shield mechanic, Belial has several attacks that can quickly spiral out of control if you are not paying attention.
He uses a heavy arm slam that sends out multiple waves of poison from the point of impact. He also has a sweeping melee attack that deals physical damage and can make targets Vulnerable. His poison breath can sweep across the arena, leaving behind poison pools that limit movement and punish sloppy positioning. He can also channel additional effects that create even more poison zones, turning the arena into a slow death trap over time.
The most dangerous mechanic, though, is the eye pattern. At certain points, Belial summons seven eyes around himself. Most of them open and prepare a large area explosion, but one or two remain black. Those black eyes mark the safe zones. If you identify them fast enough, you can survive the attack. If you hesitate, you are probably dead.
Because of that, Poison Resistance and strong situational awareness are extremely important in this fight. If your build cannot comfortably hit 70% Poison Resistance cap, using an Elixir of Poison Resistance II before the fight is a smart adjustment.
Belial Loot
The biggest reason Belial matters in Season 12 is simple: his loot is excellent.
Lair Boss rewards scale with Torment level and party size, and Belial sits near the top of that reward structure. If you are farming for premium gear, he is one of the best bosses to prioritize.
